Maybe your cartridge was borked. I had it on the MegaDrive, it looked EXACTLY like this. At least that's what I remember...
Altered Beast on the Mega Drive is one of my all time favourites. It had pretty much everything I wanted in a game^^.
...Now I think about it, it still does.
